The Molina Healthcare Charitable Foundation (“MolinaCares”), in collaboration withMolina Healthcare of Missouri (“Molina”), awarded a $20,000 grant to Hope Family Care Center to help open the Trinity Temple Community Health Access Site in Grandview, Missouri as part of the Hope Family Care Center’s Hope to You Initiative.
Parts of Grandview, including where Trinity Temple is located, are primary care health professional shortage areas. Living in an area impacted by provider shortages can lead to delays in diagnosis and treatment of chronic conditions, which can cause worse health outcomes and more costly care. This initiative brings health services and care navigation into places where people live, worship and gather. Care navigators will help patients coordinate services, overcome obstacles such as transportation and stay connected to ongoing care. The site is designed to reduce common barriers to preventive and primary care by offering services in a familiar, accessible community location.
“The Hope to You Initiative is embedding screenings, health education and care navigation into Trinity Temple, where many people already gather,” said Ericka Saielli, chief executive officer of Hope Family Care Center. “With Molina’s support, we aim to complete 100 preventive screenings and connect at least 30 people to ongoing primary care over the next year.”
Grant funding supported the medical equipment and supplies needed to launch the clinic, resources for community health workers and nurses, and referrals to Hope Family Care Center’s comprehensive medical services. The clinic opened to the public on October 2, 2026. To make an appointment, visit the Hope Family Care Center Appointment Linkor call

“Access to care is about meeting Missourians where they are, in places they know and trust,” said Jay Ludlam, plan president of Molina Healthcare of Missouri. “Our partnership with Hope Family Care Center and Trinity Temple brings care and resources closer to families in a trusted community setting. Molina is proud to support this locally driven approach to expanding access and improving the health and well-being of underserved communities.”
